Office Appointments

Office appointments are available Monday through Friday.  Please call 336.716.8095 to schedule.  When calling, please have available your preferred contact information and your insurance card(s).

 

We have the most advanced imaging technology available.  This includes an open MRI scanner.

 

Complementary parking is by the front door.

 

What to bring to an appointment:

 

Patient Forms (If filled out before visit)

Insurance Card (Please have primary and secondary insurance information)

Name and Contact Information of Referring Physician (when referred)

Operative Reports from prior surgeries

If X-Ray Films, MRIs, or other studies are less than 12 months old, please bring original studies and study reports. (We prefer actual films over studies on CDs or paper prints.)

If X-Ray Films, MRIs, or other studies are greater than 12 months old, please bring study reports.
